Clinical Evaluation of Spontaneous Pneumothorax







Abstract
We studied 150 cases of spontaneous pneumothorax experienced at the department of Thoracic and Cardiovascular Surgery, Chung-Ang university Hospital, from 1981 to 1989. The data were as follows:
1) The sex ratio was 6.8:1 in male to female and the incidence as highest in the adolescence between 20-29 years old.
2) The site of pneumothorax was 5.3% in right and 42.7% in left.
3) The initial clinical symptoms were dyspnea (75.3%), chest pain (14.0%), chest discomfort (6.0%).
4) The etiological factors were tuberculosis (30.7%), COPD (6.0%) and others (62.0%).
5) The state of activity on attack was almost in the usual life (94.0%).
6) Recurrent pneumothorax were took placed in 30.7% of the cases.
7) The methods of treatment were closed thoracostomy (66.7%), open thoracotomy (27.3%), bed rest and oxygen inhalation (4.7%).
8), The indications of open thoracotomy were recurrence (48.8%), persistent air leakage (46.4%), visible bulla on chest X-ray (2.4%).
9) The operative procedures in open thoracotomy were excision of blebs or wedge resection (80.5 %), lobectomy (14.5%), decortication and blebs excision (4.9%).
